In our continuing efforts to keep everyone healthy and safe, please observe the following guidelines:
- To minimize the handling of paper forms and to expedite the check-in process, we ask that you complete the necessary forms electronically whenever possible. We are now sending links to the forms by text or email the day before your appointment. If you would prefer to complete paper forms, please click here.
- Masks are now optional, but if have symptoms of an upper respiratory illness, it would be best to schedule your appointment when you are symptom-free.
- Patients may be accompanied by only one adult in the office.
- Upon arrival at our office, please proceed to the front desk check-in at your scheduled appointment time.
